Diver Medical | Participant Questionnaire

Recreational scuba diving and freediving requires good physical and mental health. There are a few medical conditions which can be hazardous while diving, listed below. Those who have, or are predisposed to, any of these conditions, should be evaluated by a physician. This Diver Medical Participant Questionnaire provides a basis to determine if you should seek out that evaluation. If you have any concerns about your diving fitness not represented on this form, consult with your physician before diving. If you are feeling ill, avoid diving. If you think you may have a contagious disease, protect yourself and others by not participating in dive training and/ or dive activities. References to “diving” on this form encompass both recreational scuba diving and freediving. This form is principally designed as an initial medical screen for new divers, but is also appropriate for divers taking continuing education. For your safety, and that of others who may dive with you, answer all questions honestly.

This diving is the heir to a long history, which has its origins in the 60s, when the Archaeological Park of Baia was not even a dream yet. Sea World had the honor of existing when the first archaeological evidence returned to the light of day, and today the emotion of the first discoveries echoes with every dive, and is a fundamental part of our dives.

The past of this diving has given us the passion and love for our Bay, the respect for all the archaeological evidence that is everywhere in our territory, and the commitment to preserve it and make it accessible to all, to keep its history alive and give it the value it deserves.


Our great “submerged Pompeii” has given us so much, today we feel the duty to reciprocate, giving it the looks of visitors, the looks of the eyes of children full of hope, of elderly people full of experience, of disabled people who enjoy the freedom of flying underwater, of all the people who come to experience the emotion of diving into the past, to rejoice and come out of the water with a big smile and the company of new friends made among the bubbles.

All the words in the world would be too few to describe the passion that drives us every day, the only way to really know it is to come and visit us and live it. We are waiting for you!
